Schedule of Well Child Preventive Care Visits

Wellness checkups are an essential part of maintaining the health of your child and preventing illness. They are an opportunity for you, your child, and your pediatrician to discuss important concerns and questions.

Visits can include physical measurements, patient history, sensory screenings, behavioral assessments, and planned procedures (immunizations, screenings and other tests) at the following intervals:

  • 2 to 5 Days: Newborn Initial Visit
  • 1 Month: Vaccination Overview & Vaccine Schedule, After The Shots Instructions, Staying Healthy Assessment (SHA)
  • 2, 4, 6 Months: Vaccination
  • 9 Months: Vaccination, Lab Orders (CBC, Lead, Hep B)
  • 12 Months (1 YR): Vaccination, Dental Fluoride Treatment*, iScreen Vision Photoscreen*, Tuberculosis Screening Questionnaire (PPD Placement or Lab Test), SHA
  • 15, 18, 21 Months: Vaccination, Dental Fluoride Treatment*, Modified Checklist for Autism in Toddlers Screening (MCHAT) Starting at 18 Months
  • 24 Months (2 YRS), 30 Months: Vaccination, Dental Fluoride Treatment*, Lab Orders (CBC, Lead, Hep B)

Following 3 years of age, routine physical examinations are recommended every year until 18 years of age:

  • 3 - 5 YRS: Vaccination, Dental Fluoride Treatment*, Vision Screening, Hearing Exam, SHA, Anxiety Screening Starting at 3 Years
  • 6-18 YRS: Vaccination, Vision Screening, Hearing Exam, SHA, Anxiety Screening, Depression Screening Starting at 18 Years
* Parents may opt out if insurance does not cover the service or self-pay at the time of service ($25 each).
